Medical City Fort Worth is seeking a Social Worker (MSW) for inpatient rehab and ER roles. You will perform non-clinical biopsychosocial assessments, crisis intervention, discharge planning, and resource referrals to patients and families, emphasizing dignity and patient-centered care throughout the hospital.

Collaborating with nurses, physicians, insurers, and care teams, you will address barriers to discharge, support post-acute placement, and advocate for high-risk patients across ICU,

Qualifications

• LCSW or LMSW/RCSW-I is required.
• Master's Degree in Social Work is required.
• +1 year experience or MSW internship required.

Responsibilities

• Work collaboratively with RN case managers, acute care nurses, physicians, insurance companies, and other staff to identify and address patient needs.
• Identify and address barriers to discharge and recovery (financial, emotional, psychosocial, medical, spiritual).
• Maintain knowledge of Medicare, Medicaid, and private payer regulations and processes; understand managed-care and length-of-stay management.
• Perform thorough assessments and develop discharge plans for high-risk patients with psychosocial needs.
• Coordinate and facilitate meetings with care teams and patients/families/caregivers; assist with difficult decisions respecting patient values.
• Provide psychosocial support and advocacy, including end-of-life communications and crisis interventions.

Education

LCSW or LMSW/RCSW-I
