Job summary

HCA Houston Healthcare Tomball is seeking a Social Worker (MSW) PRN to perform biopsychosocial assessments, crisis intervention and discharge planning on inpatient floors and the ED. You will collaborate with RN case managers, physicians, and payers to identify patient needs and barriers to discharge, ensuring patient- and family-centered care.

This role spans ICU, Trauma, Oncology, NICU, Transplant and ED with emphasis on complex transitions.

Qualifications

• Masters Degree required.
• Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker/Registered Clinical Social Worker-Intern (LMSW/RCSW-I) required.

Responsibilities

• Collaborate with RN case managers, nurses, physicians, insurance companies and staff to identify and address patient needs.
• Identify and address barriers to discharge and recovery (financial, emotional, psychosocial, medical, spiritual).
• Maintain knowledge of Medicare, Medicaid and private payer regulations and managed-care concepts related to discharge planning and length of stay.
• Perform assessments and develop discharge plans for high-risk patients with psychosocial needs.
• Coordinate and facilitate meetings between care team, patients, families and caregivers.
• Intervene for psychosocial needs and support, including end-of-life, crisis intervention and resource referrals.
• Provide emotional support to patients and families during emotionally charged issues.
