Medical City Plano is seeking a Social Worker (MSW) to conduct non-clinical assessments and assist with discharge planning across inpatient and ED settings. You will collaborate with care teams, facilitate family meetings, and connect patients with financial and community resources.

A TX LCSW or MSW license is preferred, with 1+ year medical social work experience. This full-time day-shift role offers a supportive environment, ongoing training, and a comprehensive benefits package.

Qualificações

• Master of Social Work (MSW) required.
• Currently licensed as a Registered Master of Social Work in the State of TX.
• Certification in Social Work preferred.
• Licensed Clinical Social Worker preferred.
• Minimum of 1+ years medical social work experience.

Responsabilidades

• Collaborate with RN case managers, nurses, physicians, and insurers to identify patient needs.
• Identify and address barriers to discharge and recovery (financial, emotional, psychosocial, medical, spiritual).
• Maintain knowledge of Medicare/Medicaid and private payer regulations and managed-care concepts.
• Perform thorough assessments and develop discharge plans for high-risk patients.
• Coordinate and facilitate meetings with care team, patients, families, and caregivers.
• Provide psychosocial support and resources to meet emotional, social, and financial needs.
