Job summary

HCA HealthONE is seeking a Social Worker (MSW) for inpatient floors and the outpatient ED, delivering biopsychosocial assessments, crisis intervention, and discharge planning with a patient- and family-centered approach.

The MSW will coordinate with RN case managers, physicians, and insurers to address barriers to discharge and coordinate post-acute placement, while supporting emotional and financial needs of patients and families.

Qualifications

• MSW required for non-clinical psychosocial assessments and interventions.
• Experience or internship in social work helpful for discharge planning and crisis intervention.
• Knowledge of Medicare/Medicaid/private payer regulations and managed-care concepts.

Responsibilities

• Perform biopsychosocial assessments for inpatients and ED patients.
• Develop discharge plans and coordinate with care teams and families.
• Provide crisis intervention and emotional support to patients and families.
• Identify barriers to discharge and assist with post-acute placements.
• Educate and refer for financial assistance or community resources.
