HCA Florida Mercy Hospital in Miami seeks a Social Worker (MSW) to perform non-clinical, biopsychosocial assessments and interventions on inpatient floors and ED. You will engage in crisis intervention, discharge planning and referrals for financial assistance to support patients and families.

You will coordinate with care teams, advocate for patient preferences, and assist in post-acute placement, with emphasis on high-risk patients and diverse clinical settings within the hospital.

Formación

• MSW required.
• 1+ years' experience or MSW internship required.
• Licensed Clinical Social Worker preferred. Certification in Social Work preferred.
• Critical thinking, service excellence and good interpersonal skills.
• Ability to read and comprehend written/verbal instruction; PC skills.

Responsabilidades

• Collaborate with RNs, case managers, physicians, insurers and staff to identify patient needs.
• Identify and address barriers to discharge and recovery across financial, emotional, psychosocial, medical and spiritual domains.
• Maintain knowledge of Medicare/Medicaid and private payer regulations and managed-care concepts.
• Assess high-risk patients and develop discharge plans with psychosocial needs.
• Coordinate meetings between care team, patients, families and caregivers; support difficult decisions aligned with patient values.
• Provide psychosocial support and counseling throughout end-of-life, crisis situations and discharge planning.
