HCA Florida JFK North Hospital in West Palm Beach, FL seeks an MSW for non-clinical biopsychosocial assessments and discharge planning across inpatient floors and the ED. The Social Worker will coordinate care with RN case managers, physicians, and families to address barriers and support post-acute needs.

The role emphasizes crisis intervention, emotional support, and collaboration to ensure patient-centered care across units including ICU, Trauma, Oncology, NICU, Transplant and ED.

Qualifications

• Master's degree in social work or equivalent required.
• Non-clinical biopsychosocial assessments and interventions experience preferred.
• Knowledge of Medicare/Medicaid and private payer processes is a plus.

Responsibilities

• Conduct biopsychosocial assessments for inpatients and ED patients.
• Perform crisis intervention and provide emotional support to patients and families.
• Coordinate discharge planning and post-acute care placement.
• Identify barriers to discharge (financial, psychosocial, medical, spiritual).
• Collaborate with care team to develop discharge plans for high-risk patients.
• Provide advocacy, counseling, and resource referrals as needed.
