Mission Hospital, part of HCA Healthcare, Asheville, NC, is seeking a Social Worker (MSW) to perform biopsychosocial assessments, crisis intervention, and discharge planning across inpatient and ED settings. The role emphasizes patient/family-centered care, coordination with the care team, and utilization of post-acute resources and community services.

Required: Master’s in Social Work and 1+ years' experience or MSW internship; LCWS preferred.

Formación

• Master’s degree in social work.
• 1+ years’ experience or MSW internship.
• Licensed Clinical Social Worker (LCWS) preferred.

Responsabilidades

• Collaborate with care team to identify and address patient needs.
• Address barriers to discharge including financial, emotional, psychosocial, medical, and spiritual.
• Develop discharge plans for high-risk patients.
• Coordinate and facilitate meetings with patients and families.
• Provide education on community resources and post-acute care.
• Engage in community outreach and program development.
