HCA HealthONE Swedish in the Denver metro area seeks a Social Worker (MSW) to perform non-clinical biopsychosocial assessments and interventions for inpatient floors and the outpatient emergency department. The MSW conducts crisis intervention, patient/family support, high-risk screening, and referrals for financial assistance or other resource needs.

Qualifications

• MSW degree required or pursuing internship.
• 1+ year experience or MSW internship required.

Responsibilities

• Collaborate with RN case managers, nurses, physicians, insurers, and staff to identify and address patient needs.
• Identify and address barriers to discharge and recovery (financial, emotional, psychosocial, medical, spiritual).
• Maintain knowledge of Medicare/Medicaid and private payer regulations and processes.
• Perform thorough assessments and develop discharge plans for high-risk patients with psychosocial needs.
• Coordinate and facilitate meetings between the care team and patients/families/caregivers; support shared decision making.
• Provide psychosocial support and crisis intervention for end-of-life and other challenging scenarios.
