Job summary

Transylvania Regional Hospital in Brevard, North Carolina is seeking a Social Worker (MSW) to perform non-clinical biopsychosocial assessments and interventions across inpatient floors and ED. The MSW supports discharge planning, crisis intervention, and referrals to resources for patients and families.

You will work with care teams to address barriers to discharge, coordinate meetings, and provide compassionate, patient- and family-centered care throughout high-stress situations.

Qualifications

• Master’s degree in social work required.
• 1+ years’ experience or MSW internship required.
• Preferred: Licensed Clinical Social Worker (LCWS).

Responsibilities

• Collaborate with RN case managers, nurses, physicians and payers to identify patient needs.
• Identify and address barriers to discharge and recovery (financial, emotional, psychosocial, medical, spiritual).
• Maintain knowledge of Medicare, Medicaid and private payer regulations and processes.
• Perform thorough assessments and develop discharge plans for high-risk patients with psychosocial needs.
• Coordinate meetings between the care team and patients/families/caregivers, supporting informed decisions.
