HCA Florida Lake Monroe Hospital in Sanford is seeking a Social Worker (MSW) to deliver non-clinical biopsychosocial assessments and crisis interventions across inpatient floors and the emergency department. The MSW will support discharge planning, post-acute placement, and referrals for financial or resource needs.

• MSW degree required.
• Experience in inpatient and ED social work is beneficial.
• Strong knowledge of patient discharge planning and resource coordination.

Responsabilidades

• Perform biopsychosocial assessments and crisis intervention on inpatient floors and ED.
• Identify barriers to discharge (financial, emotional, psychosocial, medical, spiritual) and develop discharge plans.
• Coordinate meetings between care teams and families/caregivers and support shared decision-making.
• Provide emotional support to patients and families during end-of-life or crisis situations.
• Maintain knowledge of Medicare/Medicaid and private payer regulations and processes.
