Job summary

Advocate Health in Charlotte, NC is seeking a Licensed Social Worker (MSW) to join the care management team. The role focuses on discharge planning, patient counseling, and coordination with physicians, case managers, and other health professionals to ensure safe, timely hospital discharges.

You will assess discharge needs, document plans, participate in patient care conferences, and provide education to patients and families.

Qualifications

• MSW from an accredited school of Social Work with hospital field experience.
• Must adhere to NASW code of ethics and standards.
• Demonstrates competence in counseling and discharge planning.

Responsibilities

• Assess patients for discharge planning needs.
• Complete initial evaluation and place in medical record.
• Collaborate with Physicians and other hospital disciplines to develop plans for continued care after discharge.
• Coordinate with Physicians, HMO, PPO, Case Managers, Physical Therapy, Nursing and other personnel regarding discharge needs.
• Develop and update the discharge plan and document the plan in the medical record.
• Act as community liaison to arrange resources and enhance safe and timely discharges.
• Organize and attend patient care conferences to identify discharge needs.
• Establish and maintain positive working relationships with all referral sources.
