HCA Florida Mercy Hospital in Miami is seeking an experienced Social Worker (MSW) to perform biopsychosocial assessments and discharge planning for inpatient and ED patients. The role collaborates with care teams, identifies barriers to discharge, and helps families navigate financial and social needs.

The MSW performs crisis intervention, provides emotional support, and coordinates post‑acute care placements while prioritizing patient and family preferences and values.

Qualifications

• Masters of Social Work (MSW) required.
• 1+ years' experience or MSW internship required.
• Licensed Clinical Social Worker preferred; Certification in Social Work preferred.

Responsabilités

• Collaborate with RN case managers, nurses, physicians, insurers, and staff to identify patient needs.
• Identify and address barriers to discharge, including financial, emotional, psychosocial, medical, and spiritual factors.
• Maintain knowledge of Medicare, Medicaid, and private payer regulations and length‑of‑stay management.
