Job summary

AccentCare, Inc. is seeking a full-time Hospice Social Worker (MSW) in San Diego, CA.

The role focuses on assessing social, emotional, and spiritual needs, developing care plans with the interdisciplinary team, and guiding patients and families through advance directives and discharge planning. This position requires an MSW and licensure to practice in the state, with 1+ year of healthcare experience preferred.

Qualifications

• Master’s Degree from accredited school of Social Work or related field.
• Licensure to practice as a clinical social worker in the state of agency operation if required.
• At least one year of professional social work experience (post-graduate) in a healthcare setting; 2+ years preferred.
• Experience or education in grief counseling preferred.
• Discharge planning experience preferred.

Responsibilities

• Assist the core members of the pre-hospice/hospice team in understanding social, spiritual, and emotional factors to establish a plan of care.
• Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients.
• Assess social, spiritual, and emotional needs to estimate capacity and coping with terminal illness.
• Educate patient and family on advance directives/advanced care planning.
• Utilize community resources to support patient and family within illness limitations.
• Support the bereavement program.
• Provide discharge planning related to change of level-of-care or location-of-care.
