Mission Healthcare is seeking a Social Worker (MSW) in San Diego, CA to join our team providing compassionate social work services within home health and hospice settings. The role involves psychosocial assessments, care planning, and family support, with an emphasis on advance care planning and end-of-life discussions.

Qualified candidates will have an MSW, at least one year in healthcare, and a driver’s license.

Qualifications

• Minimum of one year of experience in health care.
• Experience in hospice or home health care strongly preferred.
• Valid BLS, CPR certification may be required or preferred.

Responsibilities

• Assess the psychosocial status of patients related to the patient’s illness and environment.
• Collaborate with the interdisciplinary team to develop and implement personalized care plans.
• Help patients and families with advance care planning and goals for end-of-life care.
• Maintain clinical records on all patients referred to social work.
• Provide information and referral services for patients and families.
