Job summary

Providence PACE seeks a social work professional to deliver services to participants in Everett. You will perform direct counseling, contribute to care planning, and help connect participants with community resources.

The role emphasizes geriatric care, cultural sensitivity, and teamwork. The position requires a Master’s Degree in Social Work and appropriate Washington state licenses, plus experience with geriatric populations.

Qualifications

Master's Degree in Social Work.
WA Clinical Independent Social Worker Associate License or WA Clinical Independent Social Worker Associate Temporary License or WA Clinical Independent Social Worker License or WA Clinical Independent Social Worker Temporary License.
2 years social work with geriatric age groups from diverse ethnic cultures.
1 year work experience with geriatric population.

Responsibilities

Provide direct social work services in the Center, including one-on-one and group counseling.
Collaborate with interdisciplinary team to address social, emotional, cognitive and ethnic factors in care planning.
Access community and financial resources and ensure continuity of care across the continuum.
Conduct social work assessment and intervention in participant homes as needed.
