Job summary

Providence PACE is seeking a social worker to provide direct and group counseling, coordinating care for geriatric participants. You will assess social, emotional, cognitive and ethnic factors to shape individualized care plans and link participants with community resources.

This role emphasizes continuity of care across settings and requires engagement with a multidisciplinary team. The position is located on-site in Everett, WA, and involves day shifts in a full-time, clinical-care capacity.

Qualifications

• Master's Degree in Social Work required.
• WA license as Social Worker required (various licenses listed).
• 2 years social work with geriatric age groups from diverse ethnic cultures.
• 1 year work experience with geriatric population.

Responsibilities

• Provide social work services in direct care and group settings within the Center.
• Assess social, emotional, cognitive and ethnic factors for care planning.
• Access community resources and ensure continuity of care across settings.
• Conduct social work assessment and intervention in other care settings as needed.
