Qualifications

• Master's Degree in Social Work required.
• Washington Clinical Independent Social Worker Associate License upon hire (or temporary license).
• Washington Clinical Independent Social Worker License upon hire (or temporary license).
• 1 year of work experience with geriatric population.
• 2 years of Social work with geriatric age groups from diverse ethnic cultures.

Responsibilities

• Provide social work services including one-on-one and group counseling.
• Conduct social work assessments and develop individualized care plans.
• Access community resources and ensure continuity of care across settings.
• Perform home visits and support families as needed.
