Formación

• Master's Degree in Social Work is required.
• Licenses for Washington Clinical Independent Social Worker are required upon hire or with temporary license options.
• Geriatric population experience required (1 year) and 2 years with diverse ethnic cultures.

Responsabilidades

• Provide social work services with direct one-on-one and group counseling.
• Address social, emotional, cognitive and ethnic factors in care plans.
• Access community and financial resources; ensure continuity of care across continuum.
• Conduct social work assessments and interventions in home or other care settings as needed.
