Qualifications

• Master's Degree in Social Work.
• Upon hire: Washington Clinical Independent Social Worker Associate License or

• Upon hire: Washington Clinical Independent Social Worker License or
• Upon hire: Washington Clinical Independent Social Worker Temporary License.
• 2 years Social work with geriatric age groups from diverse ethnic cultures.
• 1 year Work experience with geriatric population.

Responsibilities

• Provides social work services to participants, including one-on-one and group counseling.
• Assesses social, emotional, cognitive and ethnic factors for an individualized care plan.
• Accesses community and financial resources and ensures continuity of care.
• Conducts social work assessment and intervention in home and other settings as needed.
