Providence PACE is seeking a part-time social worker to join our interdisciplinary team in Western Washington. The role involves 32 hours per week on a day shift, providing direct counseling and support to older adults in community settings and clinics.

Your duties include comprehensive assessments, developing individualized care plans, and linking participants to community resources to promote independence and quality of life. Experience with geriatric populations is essential.

Qualifications

• Master's Degree in Social Work required.
• Washington Clinical Independent Social Worker licenses or temporary licenses on hire.
• 1 year of work experience with geriatric population.
• 2 years of social work with geriatric age groups from diverse ethnic cultures.

Responsabilités

• Provide social work services with direct counseling, and team collaboration.
• Assess social, emotional, cognitive and ethnic factors to plan care.
• Access community resources and ensure continuity of care across settings.
• Conduct social work assessments and interventions including home visits as needed.
