Job summary

Kaiser Permanente is seeking an experienced MSW-based social worker to coordinate inpatient case management, collaborate with the health care team, and help members access cost-effective services aligned with their care plans.

Responsibilities include psychosocial assessments, developing plans of care, and documentation, while pursuing licensure in MD/VA/DC. Occasional family meetings and travel may be required.

Qualifications

• MSW required and licensed or eligible in MD/VA/DC.
• Experience in health care setting preferred.
• Knowledge of community resources and care coordination.

Responsibilities

• Collaborate with health care team to coordinate cost-effective services.
• Complete psychosocial assessments and develop plans of care.
• Document case management activities in accordance with standards.
• Identify and facilitate access to community resources and programs.
