Job summary

Kaiser Permanente Washington Hospital Center UR-Discharge Planning team seeks a part-time social worker to coordinate discharge planning and care transitions. You will collaborate with the health care team to assess needs, arrange resources, and promote patient independence and quality of life.

The role requires an MSW and multi-state DC/MD/VA credentials within defined timelines, with inpatient duties and weekend coverage as needed. Documentation and ethical standards are emphasized.

Qualifications

• MSW required and licensed or eligible for DC/MD/VA credentials within 6 months.
• Experience in health care social work and discharge planning preferred.
• Knowledge of NASW Code of Ethics and community resources.

Responsibilities

• Collaborates with the care team to coordinate cost-effective services aligned with the plan of care.
• Performs psychosocial assessments and documents care plans and outcomes.
• Facilitates access to local, state, and federal programs and community resources.
• Assists with guardianship paperwork and family meetings as needed.
• Provides supportive counseling and education to patients, families, and staff.
• Ensures accurate and consistent documentation of case management activities.
