• Manage and coordinate a caseload with established criteria; complete psychosocial assessment.
• Develop and monitor a plan of care with the nurse case manager to optimize functioning.
• Identify and facilitate access to relevant community resources and programs.
• Document case management activities in accordance with standards.
• Analyze outcomes to identify improvements in program quality and cost effectiveness.
• Assist with applying for local, state, and federal programs and refer to agencies when needed.
• Provide supportive counseling and education to members, families and care teams, including end-of-life issues.
