Job summary

Northern Nevada State Veterans Home is seeking an experienced MDS Coordinator to lead the assessment and care planning process for residents. If you are detail-oriented, clinically skilled, and committed to accurate documentation, you’ll be a strong fit for our team.

You will own the assessment process, ensure compliant interdisciplinary care planning, and collaborate with nursing, therapy, and administrative staff to support quality outcomes and survey readiness.

Qualifications

• Current RN license in the state of employment required.
• RAC-CT certification preferred or willingness to obtain within designated timeframe.
• Prior experience in MDS coordination within a skilled nursing setting required.
• Strong knowledge of RAI process, PDPM, Medicare guidelines, and care planning.
• Excellent critical thinking, time management, and communication skills.

Responsibilities

• Own the Assessment Process: Complete timely and accurate MDS assessments in compliance with federal, state, and community guidelines.
• Drive Clinical Excellence & Compliance: Monitor clinical documentation for accuracy and support staff in completing assessments and charting.
• Lead & Collaborate: Serve as the go-to expert on MDS processes and educate team members on documentation best practices.
