Job summary

West Hollywood Healthcare & Wellness Center is seeking an experienced MDS Coordinator to join our clinical team. LVN or RN candidates are welcome to apply and will coordinate and complete MDS assessments in line with CMS and state regulations.

The role emphasizes accuracy, interdisciplinary collaboration, and proactive documentation to support resident-centered care and compliant reimbursement.

Qualifications

• Current, active LVN/LPN or RN license in the applicable state.
• Experience in skilled nursing, long-term care, or related healthcare setting.
• MDS experience strongly preferred.
• Knowledge of CMS/RAI requirements and long-term care regulations.
• Strong clinical assessment and documentation skills.
• Excellent organization and attention to detail.
• Ability to meet deadlines and manage multiple priorities.

Responsibilities

• Coordinate and complete MDS assessments per CMS/state/regulatory requirements.
• Ensure MDS assessments are accurate, timely, and well-documented.
• Coordinate interdisciplinary team (IDT) assessment and care planning.
• Review clinical records, nursing documents, therapy notes, physician docs, and more.
• Develop and update comprehensive care plans from assessments.
• Collaborate with nursing, rehab, dietary, social services, physicians, and other IDT members.
• Monitor MDS schedules and ensure timely completion.
• Participate in resident care plan meetings and communicate with residents and families.
• Support documentation and reimbursement related to Medicare/Medicaid.

Position: MDS Coordinator

License: LVN or RN
Schedule: Full-Time

What You’ll Do

• Coordinate and complete MDS assessments in accordance with current CMS, state, and regulatory requirements.

• Ensure MDS assessments are accurate, timely, and supported by appropriate clinical documentation.

• Coordinate the interdisciplinary team (IDT) assessment and care planning process.

• Review clinical records, nursing documentation, therapy notes, physician documentation, and other relevant information.

• Develop and update comprehensive care plans based on resident assessments and identified needs.

• Collaborate closely with nursing, rehabilitation, dietary, social services, activities, physicians, and other members of the IDT.

• Monitor MDS schedules and ensure assessments are completed within required timeframes.

• Participate in resident care plan meetings and communicate effectively with residents and families.

• Support accurate documentation and appropriate reimbursement related to Medicare/Medicaid services.

• Assist with audits, surveys, and regulatory compliance activities.

• Participate in QAPI initiatives and identify opportunities to improve clinical documentation and resident outcomes.

• Provide education and guidance to staff regarding MDS requirements and documentation standards.

What We’re Looking For

• Current, active LVN/LPN or RN licensein the applicable state.

• Previous experience in skilled nursing, long-term care, rehabilitation, or a related healthcare setting.

• Previous MDS experience strongly preferred.

• Knowledge of CMS/RAI requirements and long-term care regulations preferred.

• Strong clinical assessment and documentation skills.
