Job summary

Powerback Rehabilitation To You is seeking a Speech Language Pathologist to provide outpatient rehabilitation predominantly in patients' homes or in living facilities across Enfield, Granby, and Suffield. Qualifications

• Must have a valid Speech Language Pathologist license in the state(s) of practice, or proof of license eligibility.
• Must have a valid driver's license or reliable, independent transportation to and from the patients' residence.

Responsibilities

• Provide outpatient rehabilitation services in-home for older adults, often in private homes or facilities.
• Organize and deliver speech-language pathology services to facilitate rehabilitation and restore communication and swallowing functions.
• Supervise CFYs, students, and support personnel as required by licensure and standards.
• Coordinate care with the rehabilitation team and physicians to select appropriate therapeutic regimens.
• Develop and adjust plans of care, ensuring safety and adherence to ethical guidelines.
