Job summary

Powerback Rehabilitation is seeking a Speech Language Pathologist to provide in-home outpatient therapy across Enfield, Granby, Suffield, and surrounding CT areas. You will assess, treat, and document therapy plans in patients' homes and in facilities, supporting functional communication and safe swallowing.

The role offers a flexible schedule, mentorship for new grads, and opportunities to grow with Medbridge CEUs, plus a pathway for visa sponsorship and relocation assistance.

Qualifications

• Valid Speech Language Pathologist license in the state(s) of practice or license eligibility.
• Must have a valid driver’s license or reliable transportation to patients’ residences.

Responsibilities

• Supervise CFYs, students, and support personnel as needed.
• Adhere to department and patient treatment schedules; complete timely documentation.
• Attend care conferences and collaborate with the rehabilitation team.
• Refer to other disciplines to address patient needs.
• Adapt plans of care based on patient goals and progress.
• Promote safe environments and maintain equipment as required.
• Coordinate care in home and community with the team.
• Perform other related duties as required.
