Powerback Rehabilitation To You seeks a Speech Language Pathologist to provide outpatient rehabilitation in the home and in selected care facilities for older adults. You will evaluate and treat communication and swallowing disorders, coordinate with caregivers, and contribute to the therapy plan while maintaining high-quality documentation.

Responsibilities

• Provide outpatient rehabilitation services in the home or care facility setting for older adults.
• Supervise CFYs, students, and support personnel as required by licensure.
• Maintain documentation for clinical and billing purposes and participate in meetings.
• Collaborate with rehabilitation team and physician to select appropriate therapy.
• Adapt care plans to individual patient needs and safety considerations.
• Coordinate care in home and community with interdisciplinary team.
