Job summary

Commonwealth Care Alliance in Massachusetts seeks an Integrated Care Clinical Manager (RN) to coordinate care for dually-eligible enrollees with complex medical, behavioral, and social needs. You will conduct home visits, lead the ICT, and collaborate with providers to improve outcomes.

The role emphasizes community-based care, crisis assessment, and building holistic care plans with diverse stakeholders. Travel within MA required.

Qualifications

• AS in Nursing from an accredited program.
• Registered Nurse with ability to independently serve people with complex medical, behavioral, and social needs.
• Prior experience in care coordination or case management preferred.
• Medicaid/Medicare managed care experience preferred.

Responsibilities

• Engage enrollees in home visits to establish complex care management.
• Liaise between healthcare providers, community resources, and enrollees for seamless transitions.
• Perform required assessments (e.g., comprehensive assessments, MDS-HC) in a timely manner.
• Engage enrollees in care plan development and implementation; provide updates as status changes.
• Lead the interdisciplinary care team to create holistic care plans addressing medical and social needs.
• Oversee enrollee utilization of long-term services and supports to enable living in preferred location.
• Educate members about benefits, services, chronic conditions, and adherence; promote self-management.
• Follow up after hospitalizations to ensure continuity of care and prevent readmissions.
