OSF HealthCare is looking for a dynamic Primary Care Provider to join our Mobile Community Care team in Peoria, Illinois. This role is essential in providing high-quality, safe patient care to underserved communities, utilizing both in-person and virtual methods.

The successful candidate will engage with patients across various demographics, and requirements include a Master's degree in advanced practice nursing and active licensure. Benefits include generous paid time off, CME time, and health plans.

Qualifications

• Demonstrated knowledge and skills for appropriate patient care across ages served.
• IL Advanced Prac Nurse, IL Reg Nurse, Fed Controlled Sub‑Lic, BLS/ACLS Certified.

Responsibilities

• Provide in‑person and virtual care to rural underserved communities.
• Serve as the Primary Care Provider.
• Collaborate with nursing to enable 24/7 access to care.
