Job summary

Powerback Rehabilitation, LLC is seeking a Physical Therapist Assistant to join our mobile outpatient therapy team delivering in-home rehabilitation services to older adults across Connecticut. This full-time role offers a flexible schedule and the opportunity to work directly with patients in their homes, supporting independence and safe daily functioning.

You will operate under a licensed physical therapist, perform treatments, document care, and participate in ongoing mentorship and CEU

Qualifications

• Must have a valid PTA license in CT or be eligible for licensure.
• Must have a valid driver's license or reliable transportation to and from patient homes.
• Experience in outpatient or home health settings preferred.

Responsibilities

• Provide outpatient rehabilitation services in home and community settings.
• Evaluate, treat and document patient progress under the supervision of a licensed physical therapist.
• Contribute to care planning and patient education for safety and independence.
• Maintain timely and accurate documentation and scheduling in the patient record.
• Ensure equipment maintenance, safety, and infection control in patients' homes.
• Collaborate with rehab team and families to optimize patient outcomes.
