Powerback Rehabilitation, LLC is seeking a Physical Therapist Assistant for full-time mobile outpatient therapy in CT, serving Granby, Bloomfield, and Windsor. You will support the physical therapy team under the supervision of a licensed PT to deliver individualized care.

Qualifications

• Must have a valid Physical Therapist Assistant license in the state(s) of practice or eligible for licensure.
• Must have a valid driver's license or reliable transportation to patient residences.

Responsibilities

• Attends and contributes to patient care under the direction of the Physical Therapist.
• Adheres to department and patient treatment schedules and documents accordingly.
• Collaborates with rehabilitation team to implement patient-specific programs.
• Assists the Physical Therapist during tests, evaluations, and complex procedures.
• Educates patients and families on the physical therapy program under supervision.
• Maintains timely and accurate documentation for all patients treated.
• Identifies and reports equipment needs and maintenance.
• Prioritizes patient service with compassionate care.
• Executes individualized treatment plans and therapy modalities.
• Performs other related duties as required.
