Powerback is seeking a Physical Therapist Assistant for a full-time mobile outpatient therapy role covering Granby, CT and surrounding areas. The position supports a range of settings including ALF/ILF and community care, under the supervision of a physical therapist.

Qualifications

• Must have a valid license in the state of practice or be eligible for licensure.
• Must have a valid driver's license or reliable transportation to patient residences.

Responsibilities

• Attends and contributes to patient care, staffing conferences and other required meetings as a representative of physical therapy under the direction of the physical therapist.
• Adheres to department and patient treatment schedules and completes timely clinical and billing documentation.
• Works with rehabilitation team to implement programs suitable for each patient's needs.
• Assists the Physical Therapist during tests, evaluations and complex procedures.
• Instructs patients and caregivers in treatment techniques under supervision.
• Maintains documentation for all patients.
• Cleans and maintains equipment and reports repairs as needed.
• Delivers patient-centered care prioritizing quality service and compassionate support.
• Implements individualized treatment plans and provides ongoing physical therapy interventions.
