Job summary

Rehab Without Walls Neuro Rehabilitation is seeking a qualified Physical Therapist to deliver functional rehabilitation in home and community settings in Fort Collins, CO. The role emphasizes exercises and real-life task training, with autonomous practice within a collaborative therapy team.

The position offers a flexible PRN schedule, hourly compensation, and travel mileage reimbursement, with opportunities for professional growth, continuing education, and program expansion.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license by state.
• BLS or CPR certification in accordance with state regulations.
• A minimum of one year's work experience as a physical therapist preferred.
• Demonstrates knowledge of rehabilitation techniques related to neurological injury.
• Communicates effectively and professionally in verbal and written interactions.
• Ability to lift 50 pounds; may lift up to 100 pounds with safety procedures.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess effectiveness of treatment plans; attend staffing meetings and family conferences.
• Perform discharge evaluations and provide home/community safety recommendations and resources.
• Communicate patient's needs and progress to the treatment team, physician, patient and family.
• Collaborate with other disciplines to ensure coordination of care and positive patient outcomes.
