Job summary

Rehab Without Walls Neuro Rehabilitation in Buford, GA is seeking a Physical Therapist to deliver functional rehabilitation in home and community settings, including cooking, shopping, and work tasks. You will treat clients in real-life environments, not just staged settings, and work autonomously within a collaborative team of therapists.

You will evaluate, plan, and re-assess treatments, communicate patient progress to the team, and help improve safety and independence in daily activities.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted license as Physical Therapist by the state in which practicing.
• Current Basic Life Support or CPR certification in accordance with state regulations.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly re-assess effectiveness of treatment plans and attend staffing meetings.
• Perform discharge evaluations and provide home/community safety recommendations.
• Communicate needs and progress to the treatment team, physician, and family members.
• Coordinate with other disciplines to ensure collaboration and improve patient outcomes.
