Job summary

Rehab Without Walls Neuro Rehabilitation in Denver, CO seeks a licensed Physical Therapist to provide functional rehabilitation in home and community settings. You will evaluate patients, create and modify treatment plans, and collaborate with families and a multidisciplinary team to improve daily living skills.

The role emphasizes real-life rehabilitation in kitchens, stores, restaurants, and workplaces, with flexible scheduling and reimbursement for travel.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• Current BLS or CPR certification.
• Minimum of 1 year PT experience preferred.
• Demonstrates rehabilitation techniques for neurological injuries.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly re-assess treatment effectiveness; attend staffing meetings and family conferences.
• Perform discharge evaluations and provide home/community safety recommendations.
• Communicate patient needs and progress to the treatment team and family.
• Collaborate with other disciplines to promote coordinated care and outcomes.
