Job summary

Powerback Rehabilitation seeks a qualified Physical Therapist for in-home outpatient care serving older adults across diverse settings in New Hampshire. The role emphasizes evaluating and treating patients in their homes, with flexible scheduling and coverage in multiple nearby towns.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice or license eligibility.
• Master’s degree in Physical Therapy; or Master’s in PT plus Bachelor’s degree and 5 years of PT experience.
• Must have a valid driver’s license or reliable transportation to patient residences.

Responsibilities

• Provide outpatient rehabilitation services in-home for older adults.
• Assess, develop, and deliver individualized physical therapy plans.
• Supervise PT Assistants, temporary licensees, students and support personnel as permitted by state licensure.
• Document clinical and billing records and participate in care coordination with the rehab team.
• Promote safe home environments and adapt plans for each patient’s needs.
