Job summary

Powerback Rehabilitation To You seeks a Physical Therapist to deliver in-home outpatient rehabilitation for older adults in Hanover, NH. You will evaluate, treat, and implement personalized PT plans, coordinating with a multidisciplinary team to restore function and independence in the home environment.

The role offers a flexible schedule, affiliation with a nationwide rehab network, and opportunities for mentorship, CEUs, and visa/relocation support.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master’s degree in Physical Therapy or a related field with Bachelor’s in PT.
• Must have a valid driver’s license or reliable transportation to patients’ residences.

Responsibilities

• Provide in-home outpatient rehabilitation services for older adults.
• Evaluate and treat patients, develop plans of care, and coordinate with the rehab team.
• Supervise PT assistants and support personnel where required.
• Maintain timely clinical and billing documentation and attend care conferences.
• Promote safety, order supplies, and ensure high quality patient service.
