Job summary

Powerback Rehabilitation is seeking a licensed Physical Therapist to deliver in-home outpatient rehabilitation for older adults. The role emphasizes evaluating, planning, and implementing PT programs in patients' homes or in independent living facilities, with collaboration with the rehab team.

Candidates must hold a PT license in the relevant state and have a Master’s degree in Physical Therapy or equivalent. A flexible schedule is offered in a home-based care model in New Hampshire.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master’s degree in Physical Therapy; or Master’s in a related field plus Bachelor’s in Physical Therapy; or Bachelor’s with 5 years PT experience.

Responsibilities

• Provide outpatient rehabilitation in the patient’s home or in an in-home setting.
• Develop and deliver individualized physical therapy programs.
• Supervise PT assistants, temporary licensees, students as needed.
• Maintain timely clinical and billing documentation and participate in care conferences.
• Collaborate with rehabilitation team to select appropriate therapy regimens.
• Promote safety and educate patients and caregivers for independence.
