Job summary

Ennoble Care seeks an experienced Nurse Practitioner to deliver primary and palliative care to homebound patients in Passaic and Hudson Counties, NJ. You will practice under physician supervision and manage a diverse patient panel.

The role emphasizes compassionate, patient-centered care, independent clinical decision making, and collaboration with an in-house team. A NJ NP/PA certification and 1–2 years in geriatric primary care are required; hospice experience is preferred.

Qualifications

• Board certification as NP or PA in New Jersey.
• Strong clinical skills to treat, assess, and diagnose.
• Positive demeanor and strong people skills.
• Ability to problem solve and work independently.
• 1–2 years of primary care experience with geriatric patients; hospice experience preferred.

Responsibilities

• Provide high quality primary and palliative care to homebound patients in Passaic/Hudson County.
• Perform physical exams, obtain histories, and plan care.
• Communicate effectively with office staff and clinical operations.
• Care for a diverse patient panel.
• Attend quarterly medical staff meetings.
• Develop impressions, diagnoses, and treatment plans.
• Consult with patients, families, and caregivers during visits.
• Order diagnostic testing as needed.
• Prescribe medications, supplies, and interventions.
