Ennoble Care is seeking a Nurse Practitioner to deliver primary and palliative care to homebound patients in Bucks County, PA. You will work under physician supervision, perform exams, manage chronic conditions, and coordinate care with families and caregivers.

The role requires an NP license in Pennsylvania, board certification, and 1–2 years of geriatric PCP experience. Flex scheduling and team support are provided to ensure high-quality patient care.

Qualifications

• Board-certified NP with PA state license.
• 1–2 years of primary care experience with geriatric patients.
• Ability to work independently and manage patient care.

Responsibilities

• Provide primary and palliative care to homebound patients in Bucks County, PA.
• Perform physical exams, review histories, and create care plans.
• Communicate with office staff and clinical operations across platforms.
• Foster trust with patients, families, and caregivers during visits.
• Order diagnostic tests as needed and manage medications.
