Job summary

Ennoble Care seeks an experienced Nurse Practitioner to provide primary and palliative care to homebound patients in Passaic County, NJ. You will apply medical techniques under physician supervision and care for diverse patients with chronic diseases.

Responsibilities include exams, care planning, coordinating with staff, and home visits. Must have NP/PA certification in NJ and 1–2 years in geriatric primary care, hospice experience a plus.

Qualifications

  • Board Certified with valid NP/PA certification in New Jersey.
  • Strong clinical skills and ability to diagnose, treat, and assess.
  • Positive demeanor and strong people skills.
  • Ability to problem solve and operate independently.
  • 1–2 years of primary care experience with geriatric patients (hospice a plus).

Purpose:

We are looking for an experienced Nurse Practitioner to provide primary care to homebound and palliative care patients in the Passaic County, NJ region. You will apply medical techniques and principles under the supervision of a physician and you will care for a diverse set of patients with chronic diseases.

What You'll Do:
  • Provide high quality care to primary and palliative patients in and around (Passaic County, NJ)
  • Assess patient's health by performing physical examinations; obtaining, updating, and studying medical histories and creating the best plan for care.
  • Effectively communicate with office staff and clinical operations across various internal platforms
  • Provide high quality, patient centered care to a panel of patients
  • Attend quarterly medical staff meetings
  • Develop impressions, diagnosis, and treatment plans
  • Foster trust and consult with patients, families, and caregivers during visits
  • Order diagnostic testing as medically necessary
  • Prescribe medications supplies and appropriate medically necessary interventions
What You'll Need:
  • Board Certified, with a valid NP or PA certification in the state of New Jersey
  • Strong clinical skills with the ability to accurately and confidently treat, assess, and diagnose
  • Positive demeanor and strong people skills
  • Ability to confidently problem solve and operate independently
  • 1-2 years of primary care experience with geriatric patients (hospice experience highly preferred but not required)

Salary Range:
  • $130K+ (depending on experience and other incentives)
