Ennoble Care in Hudson County, NJ seeks a Nurse Practitioner or Physician Assistant to provide home-based primary and palliative care. You will deliver comprehensive medical care to homebound patients under physician supervision, developing care plans and coordinating with families.

Ideal candidates have NJ NP/PA certification and 1–2 years of primary care experience, especially with geriatric patients. A flexible schedule and strong autonomy are offered with a supportive clinical team.

Qualifications

• Board-certified NP or PA in New Jersey
• 1–2 years of primary care experience with geriatric patients
• Strong clinical, communication, and problem-solving abilities

Responsibilities

• Provide primary and palliative care to homebound patients in Hudson County, NJ
• Perform physical examinations and develop care plans
• Order diagnostic tests as needed and prescribe medications
• Collaborate with office staff and clinical operations across platforms
• Attend quarterly medical staff meetings
• Foster trust with patients, families, and caregivers during visits
