Job summary

Lifelong is seeking a Housing Case Manager to support clients in securing and sustaining stable housing in Seattle. You will manage a caseload, conduct outreach, complete applications, and coordinate with housing providers to promote stability and health.

Responsibilities include delivering harm reduction-based support, addressing barriers to housing, and connecting clients with health, financial, and employment resources. This is a full-time, on-site role in Capitol Hill, evenings Tue–Sat.

Qualifications

• Engage with clients to build rapport and support housing stability.
• Assist with housing applications and paperwork for placements.
• Advocate for housing retention and client transitions; support goal setting.

Responsibilities

• Provide resources for health, mental health, and substance use services.
• Coordinate with property management to address housing issues.
• Maintain client confidentiality and program compliance.
• Document case notes and participate in supervision and team meetings.
