Qualifications

• 2 years of experience working with adults who are experiencing homelessness or mental illness.
• Knowledge of de-escalation skills and motivational interviewing.
• Peer Counselor credential is a plus.

Responsibilities

• Manage a caseload of up to 20 clients with mental health disorders.
• Conduct assessments and develop support and treatment plans with client participation.
• Coordinate with clinical, medical, housing, peer support, and shelter leadership.
• Advocate for client access to community resources and services; ensure rights are maintained.
• Perform outreach, referral, and engagement activities to ensure continuity of care.
