Qualifications

• High school diploma or equivalent required; bachelor's degree preferred.
• Experience: minimum of three years in social work or outreach related to harm reduction.
• Bi-lingual Spanish speaking applicants are highly desired.

Responsibilities

• Direct Client Care, Street-Based Outreach & Intensive Case Management (70%).
• Care coordination across a broad network of medical, behavioral health, housing, and social services.
• Advocate for clients at individual and systems levels to improve access and outcomes.
