Job summary

Powerback Rehabilitation To You is seeking a Physical Therapist for mobile outpatient services in Watertown, CT. This PRN role focuses on in-home outpatient rehab for older adults, evaluating and treating patients in their homes or independent living facilities.

You'll design and implement individualized therapy programs, collaborate with caregivers, and promote independence while operating in a flexible, home-based setting across the region.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy or equivalent.
• Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience.
• Degree earned outside the U.S. must be evaluated for U.S. equivalence.

Responsibilities

• Supervise Physical Therapy Assistants, temporary licensees, students and support personnel as required by state licensure.
• Ensure adherence to department and patient treatment schedules and complete timely documentation.
• Attend and contribute to patient care and staffing conferences.
• Refer to other disciplines to address comprehensive patient needs.
• Collaborate with rehabilitation team to select appropriate therapy regimens.
• Adapt plans of care to meet individual patient needs.
• Promote safety and manage equipment and supplies.
• Educate patients and caregivers to promote independence at home.
