Job summary

Powerback Rehabilitation is seeking a Physical Therapist for full-time mobile outpatient therapy in Clinton, CT. You will deliver in-home and community-based rehabilitation for older adults, developing personalized PT programs and coordinating care with the rehab team.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy; or Master's in a related field plus Bachelor's in Physical Therapy; or Bachelor's in Physical Therapy with 5 years of PT experience.
• Degrees earned outside the U.S. must be evaluated to US equivalency.
• Valid driver's license or reliable transportation to patients' residences.

Responsibilities

• Provide outpatient rehabilitation services in home or facility settings for older adults.
• Assess, plan, and deliver PT programs to restore, reinforce and enhance function.
• Coordinate with rehabilitation team and refer to other disciplines as needed.
• Educate patients and caregivers; document treatments and progress.
• Maintain equipment, ensure safety, and promote wellness during home visits.
