Job summary

CVS Health Corporation's Signify Health division seeks a full-time physician to perform in-home health evaluations across a defined coverage area. You’ll travel up to 100% of the time, conducting visits that include medical history reviews and physical examinations, with tests as ordered by health plans.

Qualifications

• Active, unrestricted medical licenses in coverage area(s).
• Ability to obtain and maintain multi-state licenses as required.
• Strong communication and clinical assessment skills.

Responsibilities

• Provide in-home health evaluations per plan and clinical workflow.
• Bridge gaps in care by coordinating with health teams and plans.
• Document assessments using iPad or equivalent workflow and tools.
