Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses in coverage area, as required.

Responsibilities

• Provide in-home health evaluations for patients.
• Travel up to 100% of the time across defined coverage area.
• Conduct health assessments including medication and medical history review.
• Utilize iPad and clinical workflow during evaluations.
